Common Questions
Typically, the waiting period is waived for the new groups enrolled in our plan while some plans do not have wait periods. Depending on your plan, subsequent enrollees (late enrollees, new hires and those currently in their waiting period) have a standard waiting period of 12 months which is applied on orthodontia and major services (if applicable), which usually includes crowns, inlays, onlays, bridges and dentures. Typically, when we receive eligibility we begin a waiting period date which takes into consideration the member’s effective date with the employer group.
Arrangements will vary by plan. Please check with your specific plan for more details. In some cases where orthodontic work was performed, we pay only for the treatment months during which you were eligible under our plan.
Orthodontic services in-progress are not covered after the customer terminates.
Orthodontic bands, brackets and wires are removed at the end of active treatment and retainers may be placed for varying lengths of time. The retainer may be fixed or removable.
For DHMO plans, any fees associated with retainers are considered to be part of the comprehensive orthodontic fee. Therefore, no additional benefit or separate fee is allowed.
PPO and Indemnity plans generally have a plan orthodontic maximum and benefits available for retention are billed separately.
